INTRODUCTION

The Municipal Administrator is the municipality's central administrative official and is responsible for coordinating, leading, and directing the administration of the municipality in line with the policies and plans authorized and created by the Mayor and Council. He will be responsible to the Mayor and Council for the overall control and management of the municipality's administration and affairs.

LIST OF REQUIREMENTS

for the following documents:

Work Permits

  • Mayors Clearance

Photocopy/Original of the following:

  1. Police Clearance/NBI;

  2. Official Receipt (OR) Shall pay at the Municipal Treasurer's Office;

  3. 2*2 picture;

  4. Cedula.

  • Occupational Permit

Photocopy/Original of the following:

  1. Endorsement letter (for citizens living outside Padre Garcia);

  2. Official Receipt (OR) Shall pay at the Municipal Treasurer's Office;

  3. 2*2 picture;

  4. Cedula;

  5. Police clearance/NBI.

  • Endorsement Letter

Original copy of the following:

  1. Barangay Clearance;

  2. Official Receipt (OR) Shall pay at the Municipal Treasurer's Office.

  • Health Card Certificate

Original copy of the following:

  1. Laboratory Result;

  2. Medical Certificate from our Municipal Health Center.


School Documents

  • Esc Certificate

Photocopy/Original of the following:

  1. Certification from your respective Barangay, including names of the parent, their job, monthly income & student's full name.

  • Unemployment Certificate

Photocopy/Original of the following:

  1. Certification from Barangay stating the parent(s) are unemployed.

Temporary/Seasonal Permit

  • Ambulant

Photocopy/Original of the following:

  1. Approved documents from MPDC office;

  2. Cedula;

  3. Official Receipt (O.R). Pay the corresponding amount at the Municipal Treasurer's Office.

  • Booth Permits

Photocopy/Original of the following:

  1. The letter request for establishing a booth that has been duly authorized;

  2. Cedula

  3. Official Receipt (O.R). Pay the corresponding amount at the Municipal Treasurer's Office.

  • Motorcade Permit

Photocopy/Original of the following:

  1. The letter request for conducting a motorcade has been duly authorized (with Route map);

  2. Official Receipt (O.R). Pay the corresponding amount at the Municipal Treasurer's Office.

  • Seasonal Permit (ex. Valentines, Simbang Gabi, etc.)

Photocopy/Original of the following:

  1. Approved documents from the MPDC office;

  2. Official Receipt (O.R). Pay the corresponding amount at the Municipal Treasurer's Office;

  3. Cedula.
